The enrollment process is very simple!
Click on the application link below -or- call a USA PayCard sales representative M-F between 9 A.M. - 6 P.M. EST at 888-459-8447 (Toll-Free). We can mail or send you an application via email.
Once your application is received, it will be processed on that same business day. You should expect your card via US mail, 5-7 business days after we receive the enrollment form from you. Your PIN mailer will then arrive by mail a day or two after the card. Also, on the same day the card is mailed, direct deposit set up instructions will be sent to the employer you listed in section C. Your direct deposit information consists of:
Type of Account: Checking
Bank Name: US Bank
Routing Number: 091000022
Account Number: This will be the 16 number on the front of your card
DO NOT use the account number on the application for direct deposit - this is only a partial account number.
Once you receive the card in the mail, you should follow up with your payroll department to make sure they set up your direct deposit to the card correctly using the information above.
You can then activate your payroll debit card by calling the toll-free number on the PayCard. This will be the final step in opening your account.
